Carers Corner
Here at Montague we have a dedicated Reception Team that are all trained in Carers and will be happy to help with any of your care needs and can offer a range of support.
Our current carers register will be updated and you will receive a call on an annual basis to update your circumstances and to ask if any support is needed from the surgery and associated services.
We are working with East Riding Council, Community Link workers and many more services that are more than happy to help patients who need support.
If you feel you are a Carer and would like to discuss in more detail, please call our reception team who will be happy to help and if they cannot help they will certainly have the navigation tools to pass you onto the most appreciate person within the surgery.
Some of the things we have in place here at the surgery to help new and continuous carers within the practice are the following:
- Carers Corner notice board within the surgery waiting room which has all the information you will need to get started
- Carers Information Packs
- New Carers First Month Review
- Carers Newsletters
- Carers referral system to East Riding Council for extra support for both carers and the cared for, including carers emergency card and emergency respite
- Flexible double appointments for carers and cared for patients to attend and discuss their needs with a member of the clinical team
- Online Services - ordering of medication, viewing patient records
- Pharmacy nomination for electronic prescriptions
- Flu vaccinations, health checks and mental health screening for the carers
We have newsletters for carers and cared for that are designed and issued on a ¼ basis. If you are a Carer and would like to sign up to this newsletter please can you send the reception an email to eryccg.receptionmontague@nhs.net to include your name, contact number and email address and we will add you to our distribution list for future issues.
Identification of who are carers
Informal (unpaid) carers look after and support a partner, a relative, a friend or neighbour who has one or more of the following:
- Age related problems
- A long term illness
- Mental health problems
- Substance misuse problems
- A disability
Caring tasks and responsibilities vary for each individual but it is important to consider the impact this has on your health and wellbeing.
Looking after someone can be very rewarding; but it can also be difficult at times. No one is born knowing how to be a Carer most people learn how to look after someone relying on common sense. Information and advice is available to help and support you as a carer.
Most people would rather not have caring responsibilities. They care because they love the person or feel there is no alternative. At times all carers can feel unable to continue with the demands placed upon them. Having the opportunity to talk to someone about additional help and support can often be the first step to making the whole situation more manageable.
When carers register with the Carers Support Service they will receive the quarterly newsletter 'We Care' and can access advice, information and support.
Carers should be recognised as individuals, with their own needs. Carer support workers will help carers deal with issues in a sensitive and confidential way.
