How to get a Fit/Sick Note

Fit Notes

If you need to request a sick note (also known as a fit note) for an illness lasting more than seven days or if you have already completed a self-certification for the first seven days, you can request one through our online consultations. This service is designed to save you time and ensure your request is processed efficiently.
Alternatively, you can call the practice on 01942366986, where our friendly team will be happy to assist you with your request and answer any questions you may have about the process.

Home visits

If you're unable to come to our surgery and need a home visit

Should you require a doctor to visit you at home you should make your request, by contacting the surgery before 1pm. The doctor may telephone you first to assess your problem and this may not always lead to a visit – you still may be asked to attend the surgery. If you have any queries, please speak to one of our reception staff who will be happy to advise you.

Flu vaccinations

During flu season, our practice takes steps to help protect your health. If you're eligible for a free flu vaccination, we will contact you directly to let you know. This vaccination is a simple and effective way to reduce your risk of catching the flu and minimise its potential complications, especially for those in high-risk groups. Once you've been notified, please contact the surgery at your earliest convenience to schedule your appointment. Our team is here to make the process as smooth as possible, ensuring you receive your vaccination in a timely manner.

Eligible for free Flu Vaccine (2024/25)
The NHS recommends flu vaccination for:
  • Everyone aged 65 years and over
  • Individuals under 65 with certain medical conditions, including children and babies over 6 months of age
  • All pregnant women
  • All children aged 2 and 3 years (provided they were aged 2 or 3 on 31 August 2024)
  • All primary school children
  • Some secondary school children (Years 7 to 11)
  • Care home residents
  • Carers
  • Those living with people who are immunocompromised
  • Frontline health and social care workers

Covid Vaccinations

The COVID-19 vaccine helps protect against COVID-19, which can be a serious or life-threatening illness. It's offered on the NHS to people at increased risk of getting seriously ill from COVID-19.
What is the COVID-19 Vaccine The COVID-19 vaccine is designed to help protect those at higher risk of becoming seriously ill from the virus. Viruses like COVID-19 change over time, and the protection from previous vaccinations can fade. That’s why it’s important to boost your immunity if you’re eligible. Getting the vaccine can:
  • lower your risk of severe symptoms
  • help you recover faster if you catch COVID-19
  • reduce your chances of needing hospital care or facing serious complications
  • protect against various strains of the COVID-19 virus

How to Nominate a Pharmacy

When you request a prescription, it can be sent electronically to a pharmacy of your choice—this is known as a "nomination." Once you nominate a pharmacy:
  • You won’t need to pick up paper prescriptions from your GP surgery.
  • Your nominated pharmacy will continue receiving your prescriptions until you update or remove this nomination.
  • Any pending prescriptions you’ve ordered may still be sent to your currently nominated pharmacy.

About the NHS App

The NHS App provides a straightforward and secure way to access various NHS services. Download the app on your smartphone or tablet from the Google play or App store. Alternatively, you can access the same services through a web browser by logging in through the NHS website. You need to prove who you are to get full access to the NHS App. With full access you can:
  • order repeat prescriptions and nominate a pharmacy where you would like to collect them
  • book and manage appointments
  • view your GP health record to see information like your allergies and medicines (if your GP has given you access to your detailed medical record, you can also see information like test results)
  • book and manage COVID-19 vaccinations
  • register your organ donation decision
  • choose how the NHS uses your data
  • view your NHS number (find out what your NHS number is)
  • use NHS 111 online to answer questions and get instant advice or medical help near you

Making a complaint

We are committed to supporting our patients and believe that most issues can be resolved through open communication. If you wish to discuss a concern with our Practice Manager, please speak with our reception team who will put you through. If the Practice Manager is unavailable, we will arrange a convenient time for them to speak with you. You can view and download a copy of our practice complaints leaflet below.

Changing your contact details

If you've moved house, or changed phone number and want to inform us you can fill submit a form by clicking here.
This will send a message to the practice who will process your request for you. The practice may be in contact should they require any further information. Alternatively, you can change your phone number and email address within the NHS app.

Travelling abroad?


If you planning on travelling abroad, our practice runs travel vaccination clinics for patients. To ensure you receive the necessary vaccinations for your trip, please plan ahead and allow at least six weeks before your departure. You can access our travel risk assessment form below.
