Introducing Salford South East PCN

The number of people living in Salford and across the country is growing. As a result, we are living longer, and many of us are living with long-term health conditions like diabetes or heart disease. In January 2019, NHS England asked all GP practices across the UK to form a local Primary Care Network (PCN) with neighbouring surgeries. Salford South East Primary Care Network is one of the five PCNs in Salford and includes 10 member GP practices with a population of 84,000 patients. Salford South East PCN supports and develops the work of current primary care services. For example, our GP practices have greater flexibility and control to provide more proactive, personalised and integrated health and social care at the right time and place. Our ten-member practices include: What are the benefits to you? We know the challenges facing the healthcare system in the long term are significant. But, working together, we can offer increased access and specialised services for patients closer to home and without the need to go to the hospital. Services such as:
  • Appointments with First Contact Practitioners, Care co-ordinators Social Prescribers, Mental Health Practitioners and Pharmacists
  • Additional capacity for GP and nurse appointments with local overflow hub
  • Access to GP and nurse appointments which includes evening and weekend availability, such as cervical smear and blood tests
  • PCN GP practices working together to deliver the COVID-19 and Flu vaccine

Get the right care at the right time, and right place Our website and online help and support hub are full of resources to help signpost residents to local and national health and care services. Whether an existing patient or new to the area you can use our interactive map to quickly find or register with a GP within our Salford South East PCN.

Travel Risk Assessment

Our patients all have access to our comprehensive travel health services.

We use NaTHNaC, a network for up-to-date guidance and resources about travel destinations, set up by the UK Department of Health to help British travellers care for their health abroad. We encourage you to look at this prior to your appointment, you will find travel information and recommendations on the vaccinations you need, plus a host of information on your chosen destination for safe travelling, it can be accessed through google.

It is essential that you plan ahead, we advise at least six weeks prior to travel where possible, this is to ensure that we can offer you an appropriate appointment and order any required vaccinations in time for your journey. Please note not all vaccines are available in surgery and you may be signposted to a travel clinic.

Our practice is working together with Cornerstone Medical Practice, The Willows Surgery, Pendleton Medical Centre. Salford Medical Centre, The Quays Practice, Orient Road Medical Practice, Sorrel Bank Medical Practice, Clarendon Medical Practice, and Ordsall Health Surgery to offer patients evening and weekend appointments. From the 1st October 2022 Salford South East Primary Care Networks, groups of General Practices working together to deliver key health and care services, will be increasing access to pre-bookable, routine appointments at evenings and weekends.
Enhanced access appointments may include services like cervical screening, childhood vaccination clinics and health checks. People will be able to see a member of the general practice team, a GP, nurse or other health professional. The appointments offered via enhanced access will be held at Victoria Road Surgery (9 Victoria Road, Salford, M6 8FZ) and may be with a different clinician than usual. This clinician will have access to your health records and will be able to treat you in the same way as we would. You can access these appointments via our reception team.
